چکیده

Objective: There are concerns regarding ischemic events associated with stent-assisted coil embolization. It is important to evaluate the risk factors for delayed thromboembolic events. This study evaluated risk factors after reduction to singleantiplatelet therapy 140–365 days after embolization. Methods: We conducted a retrospective and cross-sectional study of patients who underwent stent-assisted coil embolization. Sixty eight aneurysms in 67 patients with unruptured intracranial aneurysms or the chronic phase of ruptured intracranial aneurysms between July 2010 and April 2014 were evaluated. Results: Dual-antiplatelet therapy was administered for 157 ± 65 days. Three patients who were administered the single-antiplatelet agent experienced postoperative ischemic events. One patient experienced cerebral infarction 143 days after the procedure. One patient with over 50% in-stent stenosis experienced cerebral infarction 194 days after the procedure. The final patient experienced transient ischemic attack (TIA) 223 days after the procedure, and reembolization was performed 209 days after the procedure. Postoperative ischemic events occurred more frequently in severe in-stent stenosis patient (P <0.05). Conclusions: However, severe in-stent stenosis may be a risk factor after reduction to single-antiplatelet therapy, relatively few ischemic events occurred after cessation of dual-antiplatelet therapy.

Onset Time of Ischemic Events and Antiplatelet Therapy after Intracranial Stent-assisted Coil Embolization.

BACKGROUND Stent-assisted coil embolization is effective for intracranial aneurysms, especially wide-necked aneurysms; however, the optimal antiplatelet regimens for ischemic events that develop after coil embolization have not yet been established. We aimed to determine the onset time of such postoperative ischemic events and the relationship between these events and antiplatelet therapy. ME...

Subacute stent thrombosis in intracranial stenting.

BACKGROUND AND PURPOSE We sought to determine the safety of intracranial stenting with respect to subacute stent thrombosis in patients being treated with standardized antiplatelet therapy. METHODS We retrospectively evaluated the outcome of primary intracranial stenting of atherosclerotic stenoses and of stenting in coil embolization procedures in 67 patients. We focused on those cases that ...

Stent-Assisted Coil Embolization of Intracranial Aneurysms: Complications in Acutely Ruptured versus Unruptured Aneurysms.

BACKGROUND AND PURPOSE The use of stents in the setting of SAH is controversial because of concerns about the efficacy and risk of dual antiplatelet therapy. We compare complications of stent-assisted coil embolization in patients with acutely ruptured aneurysms with complications in patients with unruptured aneurysms. MATERIALS AND METHODS Between February 2007 and March 2015, 45 acutely rup...

A case of anterior communicating artery aneurysm successfully treated after a stent migration during stent assisted endovascular coil embolization

Stent migration is a complication associated with endovascular coil embolization of intracranial aneurysms. We report a case of anterior communicating artery (ACoA) aneurysm that was successfully treated after stent migration during endovascular coil embolization without retrieval of the stent. A 47-year-old man presented with sudden onset severe headache. Patient was noted to have subarachnoid...

Thromboembolic events associated with balloon-assisted coil embolization: evaluation with diffusion-weighted MR imaging.

BACKGROUND AND PURPOSE Thromboembolic events may occur during or after the treatment of intracranial aneurysms with Guglielmi detachable coils (GDCs). The purpose of this study was to determine the frequency of thromboembolic events associated with balloon-assisted coil placement and to investigate possible risk factors for these events during balloon-assisted coil placement and embolization. ...
